The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete the appointment request form below. Our scheduling coordinator will contact you to confirm your appointment.

We see patients from 7 a.m. to 4:30 p.m. daily on Mon, Tues, Wed, and Thurs.  Our hours are from 7 a.m. to 11 a.m. on Fridays.  If you are having a problem be sure to call early and we will make every effort to see you on that day. 

We have also provided an easy option for you to get your forms completed prior to your appointment time.  Visit our "Patient Login" button to access your New Patient forms, have the ability to edit your existing address, phone, or other pertinent information, and communicate with us through email with the completion of the "Release for Internet Communication" form.
