The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete the appointment request form below. Our scheduling coordinator will contact you to confirm your appointment.

We see patients from 7 a.m. to 4:30 p.m. daily on Mon, Tues, Wed, and Thurs.  Our hours are from 7 a.m. to 11 a.m. on Fridays.  If you are having a problem be sure to call early and we will make every effort to see you on that day. 

Our "Patient Login" will help you to fill out your "New Patient Forms" or update your current information, pay your bill or check on your appointment.

Communicate with us through email with the completion of the "Release for Internet Communication" form.

Please do not use this form to cancel or change an existing appointment.
